Screening for blood donation is notoriously poor, and usually consists of filling out a form with a few questions regarding habits and past medical history and a couple of basic medical tests.
Another way to get people to donate was on an "insurance" scheme in which those who required blood and were part of the scheme would be able to get blood, but would have to "replace" it somehow, be that through family or friends willing to donate on their behalf.
Obviously the direct pay for blood system may gain a lot of blood, but that blood is mostly the least desirable quality blood.
